Admissions Process

Admissions to Oak Ridge Place by Integrity Senior Housing Management Inc. is meant to be an easy process that can be completed in up to four steps for potential new tenants:

A resident or authorized representative contacts Director of Housing & Marketing to begin the admissions process. Residents of Oak Ridge Place should be 55 years of age or older, unless special arrangements have been made.

What to expect during your interaction with our Housing team:

  • Invitation to tour our community & available units.

  • Discussion on what services we provide.

  • Completing Rental Application ($40)

  • Copies of all price lists & needed documentation to finalize move in. Full discussion on fees associated with moving in.

For potential clients searching for Assisted Living Services:

Physical assessments will be conducted with prospective residents by Registered Nurse to identify needs and help devise a plan for care. We recommend that assessments may be performed in the prospective resident’s current environment. Potential resident should anticipate contact from a Registered Nurse of Oak Ridge Place within 24 hours after communicating with Director of Housing & Marketing. Assessments for Assisted Living residents are required ,as they are a vital part of the admissions process.

What to expect during your interaction with our Nursing team:

  • At the assessment a Registered Nurse will meet with prospective resident and collect health information to learn more about their overall health, needs and concerns. We will use this information to determine plan of care, & if Oak Ridge Place can meet the needs of the individual .

  • Completing Medical Release paperwork, if not done previously

  • Provided copies of price lists and our disclosures of services provided & limitations.

What to bring to your nursing appointment:

  • Copy of POLST/Health Care Directive

  • List of current medication orders signed by your doctor

  • Copy of your current Power of Attorney form.

Based on the assessment results, the Director of Housing & Marketing and Clinical Nurse Supervisor will determine what accommodations can be provided to meet the prospective resident’s needs and the approximate cost. This information will be made available to you within 72 hours of receiving medical records and required information.

Family arranges time with the admissions representative prior to the day the resident moves to Oak Ridge Place. We believe it works best for both resident and family if admission paperwork is completed and furniture is in place before the move-in date. We prefer residents move in at the beginning of any given week to ensure a smooth transition.

Please keep in mind that Oak Ridge Place is not a medically-based care model like a skilled nursing facility, but a licensed assisted living provider. As the care needs of an individual change, the registered nurse may determine that extra staff or services need to be provided at an additional cost to ensure safety and quality of care for all residents. We will make every effort to ensure that all options are examined so your loved one may remain at Oak Ridge Place. Should this be the case, you will be notified and a care conference will be scheduled to discuss further options.
