Assisted Living Services

At Oak Ridge Place, our Assisted Living services provide personalized support that helps residents maintain their independence while enjoying the comfort and security of having caring staff available when needed.

Services are based on each resident’s individual care needs and may include:

  • 24-Hour Staff Support – Caring staff are available around the clock to provide assistance and respond to resident needs.

  • Personal Care Assistance – Support with daily activities such as bathing, dressing, grooming, and toileting.

  • Medication Management – Assistance with medication administration and management according to each resident’s care plan.

  • Nursing Services – Nursing oversight, health monitoring, care coordination, and other services based on individual needs.

  • Meals – Nutritious meals are provided, with accommodations for dietary needs as appropriate.

  • Housekeeping & Laundry – Regular housekeeping and laundry services help residents maintain a clean and comfortable home.

  • Emergency Response – Residents have access to staff support in the event of an emergency or unexpected need.

  • Activities & Social Opportunities – A variety of activities and social opportunities encourage residents to stay active, engaged, and connected.

Nursing and Home Health Services

Admissions Process

Admissions to Oak Ridge Place by Integrity Senior Housing Management Inc. is meant to be an easy process that can be completed in four steps for potential Assisted Living Residents:

Admissions Process

At Oak Ridge Place Senior Living, we strive to make the admissions process as simple and welcoming as possible. Our admissions process for prospective Assisted Living residents can be completed in four steps.

Step 1: Contact Our Housing & Marketing Team

The prospective resident or their authorized representative contacts the Director of Housing & Marketing to begin the admissions process.

Residents of Oak Ridge Place should be 55 years of age or older unless special arrangements have been made.

What to Expect

During your initial conversations with our Housing & Marketing team, you can expect:

  • An invitation to tour our community and available apartments

  • Completion of a Rental Application ($40 application fee)

  • Completion of a Medical Release form

  • Copies of applicable price lists and information about the documentation needed to complete the move-in process

Step 2: Nursing Assessment

A Registered Nurse will conduct an assessment with the prospective resident to better understand their individual needs and help develop an appropriate plan of care.

Whenever possible, we recommend completing the assessment in the prospective resident's current living environment.

A Registered Nurse from Oak Ridge Place will contact the prospective resident within approximately 24 hours after the initial conversation with the Director of Housing & Marketing.

Nursing assessments are required for all prospective Assisted Living residents and are an important part of determining whether Oak Ridge Place can safely and appropriately meet the individual's needs.

Assessments will be scheduled after the prospective resident has completed and submitted an approved Rental Application.

What to Expect During the Nursing Assessment

During the assessment, a Registered Nurse will:

  • Meet with the prospective resident and gather information about their overall health, needs, and concerns

  • Review the information needed to develop an individualized plan of care

  • Determine whether Oak Ridge Place can appropriately meet the prospective resident's needs

  • Complete Medical Release paperwork, if it has not already been completed

  • Provide applicable service and pricing information

What to Have Available for Your Nursing Assessment

Please have the following available, if applicable:

  • A copy of your POLST and/or Health Care Directive

  • A current list of medication orders signed by your healthcare provider

  • A copy of your current Power of Attorney documentation

Step 3: Review of Services & Costs

Following the nursing assessment, the Director of Housing & Marketing and Clinical Nurse Supervisor will review the prospective resident's needs and determine what accommodations and services Oak Ridge Place can provide.

An approximate cost of services will also be provided.

This information is generally made available within 72 hours after Oak Ridge Place receives the necessary medical records and required information.

Step 4: Preparing for Move-In

Before the resident's move-in date, the resident and/or family will arrange a time to meet with an admissions representative to complete the necessary admission paperwork.

Whenever possible, we recommend completing admission paperwork and having furniture and personal belongings in place before the resident arrives. This helps make move-in day more comfortable and allows the resident to focus on settling into their new home.

When possible, we prefer Assisted Living residents to move in near the beginning of the week to help ensure a smooth transition.

As Care Needs Change

Oak Ridge Place is a licensed Assisted Living provider and does not operate under the same medically based care model as a skilled nursing facility.

As a resident's needs change, our Registered Nurse may determine that additional services or staffing are necessary to support the resident's safety and quality of care. Additional services may result in additional costs.

Our team will make every effort to explore available options that may allow the resident to continue calling Oak Ridge Place home. If a resident's needs change significantly, the resident and/or their representative will be notified, and a care conference may be scheduled to discuss available options and next steps.
